Variant summary
Our verdict is Benign. Variant got -9 ACMG points: 0P and 9B. BP4_StrongBP6BS1
The NM_006231.4(POLE):c.1015G>A(p.Asp339Asn) variant causes a missense change. The variant allele was found at a frequency of 0.0000738 in 1,598,432 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 13/21 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. D339V) has been classified as Uncertain significance.

Uncertain significance, criteria provided, single submitter | clinical testing | Genetic Services Laboratory, University of Chicago | Nov 30, 2021 | DNA sequence analysis of the POLE gene demonstrated a sequence change, c.1015G>A, in exon 10 that results in an amino acid change, p.Asp339Asn. This sequence change has been described in the gnomAD database with a frequency of 0.11% in the African/African American subpopulation (dbSNP rs149029910). The p.Asp339Asn change affects a highly conserved amino acid residue located in a domain of the POLE protein that is known to be functional. The p.Asp339Asn substitution appears to be benign using several in-silico pathogenicity prediction tools (SIFT, PolyPhen2, Align GVGD, REVEL). This sequence change does not appear to have been previously described in individuals with POLE-related disorders. Due to insufficient evidences and the lack of functional studies, the clinical significance of the p.Asp339Asn change remains unknown at this time. - |

Hereditary cancer-predisposing syndrome Benign:2
Likely benign, criteria provided, single submitter | clinical testing | Ambry Genetics | Nov 10, 2022 | This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. - |
