This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 266 of the POLE protein (p.Arg266Gln). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with colorectal cancer (PMID: 33193653). ClinVar contains an entry for this variant (Variation ID: 240629). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt POLE protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.R266Q variant (also known as c.797G>A), located in coding exon 8 of the POLE gene, results from a G to A substitution at nucleotide position 797. The arginine at codon 266 is replaced by glutamine, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -

The POLE c.797G>A (p.Arg266Gln) missense variant is absent in gnomAD v2.1.1 (PM2_Supporting; https://gnomad.broadinstitute.org/ ). In silico tools are not in agreement about the effect of this variant on protein function, but to our knowledge these predictions have not been confirmed by functional assays. This variant has been reported in one ultramutated tumor, as well as in tumors with low mutational burden (PMID: 29056344). It has also been reported in one family with history of colorectal cancer (PMID: 33193653). In summary, this variant meets criteria to be classified as of uncertain significance based on the ACMG/AMP criteria: PM2_Supporting. -
