Variant summary
Our verdict is Likely benign. The variant received -2 ACMG points: 0P and 2B. BP4_Moderate
The NM_020297.4(ABCC9):c.1982G>A(p.Arg661His) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000123 in 1,461,688 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R661C) has been classified as Uncertain significance.

Dilated cardiomyopathy 1O Uncertain:1
This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 661 of the ABCC9 protein (p.Arg661His). This variant is present in population databases (rs397517185, gnomAD 0.004%). This missense change has been observed in individual(s) with hypertrophic cardiomyopathy (PMID: 29030401). ClinVar contains an entry for this variant (Variation ID: 45394). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on ABCC9 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
not specified Uncertain:1
The Arg661His variant in ABCC9 has not been reported in the literature nor previ ously identified by our laboratory. It was also not detected in 2 large and broa d populations (European and African American) screened by the NHLBI Exome Sequen cing Project (http://evs.gs.washington.edu/EVS/), which is consistent with patho genicity but insufficient to rule out a benign role. Computational analyses (bio chemical amino acid properties, conservation, AlignGVGD, PolyPhen2, and SIFT) do not provide strong support for or against an impact to the protein. Additional information is needed to fully assess the clinical significance of the Arg661His variant. -
