Genes affected
KMT2D (HGNC:7133): (lysine methyltransferase 2D) The protein encoded by this gene is a histone methyltransferase that methylates the Lys-4 position of histone H3. The encoded protein is part of a large protein complex called ASCOM, which has been shown to be a transcriptional regulator of the beta-globin and estrogen receptor genes. Mutations in this gene have been shown to be a cause of Kabuki syndrome. [provided by RefSeq, Oct 2010]

The variant is not observed in the gnomAD v2.1.1 dataset. Predicted Consequence/Location: The majority of the known disease-causing variants of this gene are variants expected to result in premature termination of the protein. In silico tool predictions suggest damaging effect of the variant on gene or gene product [REVEL: 0.80 (>=0.6, sensitivity 0.68 and specificity 0.92); 3Cnet: 0.87 (>=0.6, sensitivity 0.72 and precision 0.9)]. Same nucleotide change resulting in same amino acid change has been previously reported as pathogenic/likely pathogenic with strong evidence (ClinVar ID: VCV000286834 /PMID: 21607748). The variant has been observed in at least two similarly affected unrelated individuals (PMID: 27302555). The variant has been previously reported as assumed (i.e. paternity and maternity not confirmed) de novo in at least one similarly affected unrelated individual (PMID: 27302555). A different missense change at the same codon (p.Arg5154Trp) has been reported to be associated with KMT2D-related disorder (ClinVar ID: VCV002579159). Therefore, this variant is classified as Pathogenic according to the recommendation of ACMG/AMP guideline. -

Not observed in large population cohorts (Lek et al., 2016); In silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect; This variant is associated with the following publications: (PMID: 21607748, 27302555, 28991257, 32368696, 30459467) -

This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 5154 of the KMT2D protein (p.Arg5154Gln). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with Kabuki syndrome (PMID: 21607748, 23913813, 25755104, 27302555, 28884922). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 286834). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on KMT2D protein function. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. For these reasons, this variant has been classified as Pathogenic. -

The KMT2D c.15461G>A variant is predicted to result in the amino acid substitution p.Arg5154Gln. This variant has been reported in individuals with Kabuki syndrome as a de novo variant and in other cases, family studies were not completed to determine inheritance of the variant (Li et al. 2011. PubMed ID: 21607748; Miyake et al. 2013. PubMed ID: 23913813; Bögershausen et al. 2016. PubMed ID: 27302555; Digilio et al. 2017. PubMed ID: 28884922). It was found de novo in a fetus with features consistent with KMT2D-associated disease (Internal Data, PreventionGenetics). This variant has not been reported in a large population database, indicating this variant is rare. This variant is interpreted as pathogenic. -
