Genes affected
AQP2 (HGNC:634): (aquaporin 2) This gene encodes a water channel protein located in the kidney collecting tubule. It belongs to the MIP/aquaporin family, some members of which are clustered together on chromosome 12q13. Mutations in this gene have been linked to autosomal dominant and recessive forms of nephrogenic diabetes insipidus. [provided by RefSeq, Oct 2008]
AQP5-AS1 (HGNC:55474): (AQP5 and AQP2 antisense RNA 2)

ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 8 ACMG points.

PVS1
Splicing +-2 bp (donor or acceptor) variant, product NOT destroyed by NMD, known LOF gene, truncates exone, which is 0.20220588 fraction of the gene. Cryptic splice site detected, with MaxEntScore 4.9, offset of 5, new splice context is: cttctctgtccccggctcAGcaa. Cryptic site results in frameshift change. If cryptic site found is not functional and variant results in exon loss, it results in inframe change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 12-49954153-A-G is Pathogenic according to our data. Variant chr12-49954153-A-G is described in ClinVar as [Likely_pathogenic]. Clinvar id is 1974447.Status of the report is criteria_provided_single_submitter, 1 stars.

This sequence change affects an acceptor splice site in intron 1 of the AQP2 gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in AQP2 are known to be pathogenic (PMID: 9024277, 27156763). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with AQP2-related conditions. ClinVar contains an entry for this variant (Variation ID: 1974447). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -
