CEP290 (HGNC:29021): (centrosomal protein 290) This gene encodes a protein with 13 putative coiled-coil domains, a region with homology to SMC chromosome segregation ATPases, six KID motifs, three tropomyosin homology domains and an ATP/GTP binding site motif A. The protein is localized to the centrosome and cilia and has sites for N-glycosylation, tyrosine sulfation, phosphorylation, N-myristoylation, and amidation. Mutations in this gene have been associated with Joubert syndrome and nephronophthisis and the presence of antibodies against this protein is associated with several forms of cancer. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 0.00029 in 245892 control chromosomes (gnomAD), predominantly at a frequency of 0.0037 within the East Asian subpopulation in the gnomAD database. The observed variant frequency within East Asian control individuals in the gnomAD database is approximately 3 fold of the estimated maximal expected allele frequency for a pathogenic variant in CEP290 causing Meckel Syndrome Type 4 (0.0011), strongly suggesting that the variant is a benign polymorphism found primarily in populations of East Asian origin. c.503G>A has been reported in the literature in individuals affected with Retinitis Pigmentosa (Jespersgaard_2019). This report does not provide unequivocal conclusions about association of the variant with Meckel Syndrome Type 4. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 30718709, 33924653). Five ClinVar submitters have assessed the variant since 2014: three classified the variant as uncertain significance, and two as likely benign. Based on the evidence outlined above, the variant was classified as likely benign. -
