Variant summary
Our verdict is Likely benign. Variant got -3 ACMG points: 2P and 5B. PM5BP4_StrongBP6
The NM_000059.4(BRCA2):āc.215A>Gā(p.Asn72Ser) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000093 in 1,613,762 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 14/19 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. N72I?) has been classified as Pathogenic.

The BRCA2 c.215A>G (p.Asn72Ser) missense variant has a maximum subpopulation frequency of 0.055% in gnomAD v2.1.1 (https://gnomad.broadinstitute.org/). This variant has been reported in the literature in both individuals with breast cancer and healthy controls (PMID: 28222693, 30287823, 30702160, 32467295, 35918668). The in silico tool REVEL predicts a benign effect on protein function, but to our knowledge this prediction has not been confirmed by functional studies. In summary, the evidence currently available is insufficient to determine the clinical significance of this variant. It has therefore been classified as of uncertain significance. -

In silico analysis supports that this missense variant does not alter protein structure/function; Also known as 443A>G; This variant is associated with the following publications: (PMID: 10923033, 18627636, 28222693, 22476429, 28873162, 30287823, 31925297, 32377563, 30702160, 30883759, 32467295, 31837001, 31853058) -

Hereditary cancer-predisposing syndrome Benign:2
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -

Variant summary: BRCA2 c.215A>G (p.Ser215Arg) results in a conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4e-05 in 251386 control chromosomes (gnomAD). This frequency is not significantly higher than expected for a pathogenic variant in BRCA2 causing Hereditary Breast And Ovarian Cancer Syndrome (4e-05 vs 0.00075), allowing no conclusion about variant significance. c.215A>G has been reported in the literature in individuals affected with Hereditary Breast And Ovarian Cancer (example, Thirthagiri_2008, Lu_2012, Lai_2017) and unaffected healthy control individuals (example Lai_2017, Momozawa_2018, and Guo_2019) . These reports do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Seven clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Five laboratories have classified the variant as unknown significance and two laboratories classified the variant as likely benign. Based on the evidence outlined above, the variant was classified as VUS. -
