Variant summary: BRCA2 c.449A>G (p.His150Arg), also known as c.677A>G (p.His150Arg), results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 251052 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.449A>G has been reported in the literature in individuals with breast cancer, as well as in at-least one individual with familial pancreatic cancer (example: Salazar_2006, Guindalini_2022, Wang_2022, Singhal_2021). These report(s) do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. This missense variant replaces histidine with arginine at codon 150 of the BRCA2 protein. Computational prediction suggests that this variant may not impact protein structure and function. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in four individuals affected with breast cancer from three families and an individual with a family history of pancreatic cancer (PMID: 15876480, 35264596, 35714671). A multifactorial analysis has reported a likelihood ratio for pathogenicity based on personal and family history of 0.636 from log(LR)=-0.196588486 for one carrier (PMID: 31853058). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity.
