The c.681+4A>T intronic variant results from an A to T substitution 4 nucleotides after coding exon 7 in the BRCA2 gene. This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration may weaken the native splice donor site, however direct evidence is insufficient at this time (Ambry internal data). A similar alteration at the same nucleotide position, c.681+4A>G, has been reported in a French family with breast and/or ovarian cancer and has been predicted to impact splicing based on a combination of functional and in silico analyses (Lecarpentier J et al. Breast Cancer Res. 2012 Jul 3;14(4):R99; Houdayer C et al. Hum Mutat. 2012 Aug;33(8):1228-38). Based on the available evidence, the clinical significance of this variant remains unclear.
Hereditary breast ovarian cancer syndrome Uncertain:1
This sequence change falls in intron 8 of the BRCA2 gene. It does not directly change the encoded amino acid sequence of the BRCA2 protein. RNA analysis indicates that this variant induces altered splicing and may result in an absent or altered protein product. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with BRCA2-related conditions. ClinVar contains an entry for this variant (Variation ID: 233034). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this variant results in skipping of exon 8, and produces a non-functional protein and/or introduces a premature termination codon (internal data). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
