Missense variant in a coldspot region where missense variants are very unlikely to be pathogenic (PMID:31911673). -
This missense variant replaces serine with arginine at codon 326 of the BRCA2 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). A functional study has reported that this variant does not impact BRCA2 function as assayed by response to PARP inhibitors (PMID: 32444794). This variant has not been reported in individuals affected with hereditary cancer in the literature. This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.S326R variant (also known as c.978C>G), located in coding exon 9 of the BRCA2 gene, results from a C to G substitution at nucleotide position 978. The serine at codon 326 is replaced by arginine, an amino acid with dissimilar properties. This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

In the published literature, this variant has been reported in affected individuals with breast and/or ovarian cancer (PMID: 22476429 (2012)), as well as in an individual with both breast cancer and ocular melanoma (PMID: 10399947 (1999)). A functional study found that this variant was not damaging to protein function (PMID: 32444794 (2020)). This variant has not been reported in large, multi-ethnic general populations (Genome Aggregation Database, http://gnomad.broadinstitute.org). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -

Variant summary: BRCA2 c.978C>G (p.Ser326Arg) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 232538 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. p.Ser326Arg has been reported in the literature in individuals affected with Breast And Ovarian Cancer Syndrome (e.g. Martin_2001, Lu_2012), however, these reports do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome since this amino acid change may also be caused by c.978C>A, which is a known polymorphism (ClinVar: 52898, benign). Protein level evidence from a PARP-inhibitor survival assay showed the variant behaved similarly to wild-type (Ikegami_2020). One ClinVar submitter has assessed the variant since 2014: the variant was classified as likely benign. Based on the evidence outlined above, the variant was classified as likely benign. -
