This variant is denoted BRCA2 c.1756A>T at the cDNA level and p.Lys586Ter (K586X) at the protein level. The substitution creates a nonsense variant, which changes a Lysine to a premature stop codon (AAA>TAA), and is predicted to cause loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay. Although this variant has not, to our knowledge, been reported in the literature, it is considered pathogenic.
This nonsense variant causes the premature termination of BRCA2 protein synthesis. The variant has not been reported in the published literature. Based on the available information, this variant is classified as pathogenic.

Variant summary: BRCA2 c.1756A>T (p.Lys586X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant was absent in 249898 control chromosomes. To our knowledge, no occurrence of c.1756A>T in individuals affected with Hereditary Breast And Ovarian Cancer Syndrome has been reported. Four clinical diagnostic laboratories and one expert panel have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic.
This sequence change creates a premature translational stop signal (p.Lys586*) in the BRCA2 gene. It is expected to result in an absent or disrupted protein product. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with BRCA2-related disease. ClinVar contains an entry for this variant (Variation ID: 231643). Loss-of-function variants in BRCA2 are known to be pathogenic (PMID: 20104584). For these reasons, this variant has been classified as Pathogenic.

The p.K586* pathogenic mutation (also known as c.1756A>T), located in coding exon 9 of the BRCA2 gene, results from an A to T substitution at nucleotide position 1756. This changes the amino acid from a lysine to a stop codon within coding exon 9. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
