Variant summary
Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong
The NM_000059.4(BRCA2):c.2471T>G(p.Leu824*) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★★). Synonymous variant affecting the same amino acid position (i.e. L824L) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

This variant is denoted BRCA2 c.2471T>G at the cDNA level and p.Leu824Ter (L824X) at the proteinlevel. Using alternate nomenclature, this variant would be defined as BRCA2 2699T>G. The substitution creates anonsense variant, which changes a Leucine to a premature stop codon (TTA>TGA), and is predicted to cause loss ofnormal protein function through either protein truncation or nonsense-mediated mRNA decay. This variant has beenreported as a pathogenic variant in one French family, however, no clinical information was provided (Lecarpentier2012). Based on currently available information, we consider this variant to be pathogenic
Breast-ovarian cancer, familial, susceptibility to, 2 Pathogenic:2
Variant allele predicted to encode a truncated non-functional protein.
Hereditary cancer-predisposing syndrome Pathogenic:1
The p.L824* pathogenic mutation (also known as c.2471T>G), located in coding exon 10 of the BRCA2 gene, results from a T to G substitution at nucleotide position 2471. This changes the amino acid from a leucine to a stop codon within coding exon 10. This mutation was previously reported in a cohort of BRCA1/2 mutation carriers that was being studied to assess potential risk-modifying factors for BRCA1/2-associated cancers (Lecarpentier J et al. Breast Cancer Res. 2012 Jul;14:R99). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
Familial cancer of breast Pathogenic:1
Hereditary breast ovarian cancer syndrome Pathogenic:1
This sequence change creates a premature translational stop signal (p.Leu824*) in the BRCA2 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in BRCA2 are known to be pathogenic (PMID: 20104584). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with BRCA2-related conditions. ClinVar contains an entry for this variant (Variation ID: 51294). For these reasons, this variant has been classified as Pathogenic.
