Genes affected
BRCA2 (HGNC:1101): (BRCA2 DNA repair associated) Inherited mutations in BRCA1 and this gene, BRCA2, confer increased lifetime risk of developing breast or ovarian cancer. Both BRCA1 and BRCA2 are involved in maintenance of genome stability, specifically the homologous recombination pathway for double-strand DNA repair. The largest exon in both genes is exon 11, which harbors the most important and frequent mutations in breast cancer patients. The BRCA2 gene was found on chromosome 13q12.3 in human. The BRCA2 protein contains several copies of a 70 aa motif called the BRC motif, and these motifs mediate binding to the RAD51 recombinase which functions in DNA repair. BRCA2 is considered a tumor suppressor gene, as tumors with BRCA2 mutations generally exhibit loss of heterozygosity (LOH) of the wild-type allele. [provided by RefSeq, May 2020]

Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsNov 22, 2023The p.L1059R variant (also known as c.3176T>G), located in coding exon 10 of the BRCA2 gene, results from a T to G substitution at nucleotide position 3176. The leucine at codon 1059 is replaced by arginine, an amino acid with dissimilar properties. This variant was identified in an individual from Dominica who was diagnosed with breast cancer at age 51 (George SHL et al. JAMA Netw Open, 2021 Mar;4:e210307). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

not provided Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingGeneDxDec 19, 2017This variant is denoted BRCA2 c.3176T>G at the cDNA level, p.Leu1059Arg (L1059R) at the protein level, and results in the change of a Leucine to an Arginine (CTG>CGG). Using alternate nomenclature, this variant would be defined as BRCA2 3404T>G. This variant has not, to our knowledge, been published in the literature as pathogenic or benign. BRCA2 Leu1059Arg was not observed at a significant allele frequency in large population cohorts (Lek 2016). Since Leucine and Arginine differ in polarity, charge, size or other properties, this is considered a non-conservative amino acid substitution. BRCA2 Leu1059Arg is located within the RAD51 interaction domain (Roy 2012). In-silico analyses, including protein predictors and evolutionary conservation, support a deleterious effect. Based on currently available information, it is unclear whether BRCA2 Leu1059Arg is pathogenic or benign. We consider it to be a variant of uncertain significance. -
Hereditary breast ovarian cancer syndrome Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingInvitaeDec 26, 2023This sequence change replaces leucine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 1059 of the BRCA2 protein (p.Leu1059Arg). This variant is present in population databases (no rsID available, gnomAD 0.007%). This missense change has been observed in individual(s) with breast cancer (PMID: 33646313). ClinVar contains an entry for this variant (Variation ID: 419515). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
not specified Benign:1
Likely benign, criteria provided, single submitterclinical testingWomen's Health and Genetics/Laboratory Corporation of America, LabCorpNov 21, 2022Variant summary: BRCA2 c.3176T>G (p.Leu1059Arg) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 8.1e-06 in 246522 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.3176T>G has been reported in the literature as a VUS in settings of multigene panel testing in at-least one woman reporting an age at first breast cancer diagnosis of 51 years (example, George_2021). These report(s) do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. At-least one co-occurrence with another pathogenic variant(s) has been reported in the UMD database (BRCA1 c.5038_5041dup, p.Thr1681AsnfsX3), providing supporting evidence for a benign role. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Four clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance but none have specified the co-occurrence captured in the context of this evaluation. Based on the evidence outlined above, the variant was classified as likely benign. -
