Genes affected
BRCA2 (HGNC:1101): (BRCA2 DNA repair associated) Inherited mutations in BRCA1 and this gene, BRCA2, confer increased lifetime risk of developing breast or ovarian cancer. Both BRCA1 and BRCA2 are involved in maintenance of genome stability, specifically the homologous recombination pathway for double-strand DNA repair. The largest exon in both genes is exon 11, which harbors the most important and frequent mutations in breast cancer patients. The BRCA2 gene was found on chromosome 13q12.3 in human. The BRCA2 protein contains several copies of a 70 aa motif called the BRC motif, and these motifs mediate binding to the RAD51 recombinase which functions in DNA repair. BRCA2 is considered a tumor suppressor gene, as tumors with BRCA2 mutations generally exhibit loss of heterozygosity (LOH) of the wild-type allele. [provided by RefSeq, May 2020]

Hereditary cancer-predisposing syndrome Uncertain:1Benign:1
Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsMar 20, 2023The p.H1731Y variant (also known as c.5191C>T), located in coding exon 10 of the BRCA2 gene, results from a C to T substitution at nucleotide position 5191. The histidine at codon 1731 is replaced by tyrosine, an amino acid with similar properties. In a study of 1854 high-risk breast and ovarian cancer families in Italy, this alteration was detected in one family (Azzollini J et al. Eur. J. Intern. Med., 2016 Jul;32:65-71). This variant has also been identified in multiple individuals diagnosed with breast cancer (Borg A et al. Hum. Mutat., 2010 Mar;31:E1200-40; Su Y et al. Front Genet, 2021 Nov;12:674094). This variant has also been reported in an individual diagnosed with breast cancer; however this individual was also positive for a different pathogenic BRCA2 mutation (Santonocito C et al. Cancers (Basel), 2020 May;12:). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Uncertain significance, criteria provided, single submitterclinical testingWomen's Health and Genetics/Laboratory Corporation of America, LabCorpSep 16, 2019Variant summary: BRCA2 c.5191C>T (p.His1731Tyr) results in a conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 8.2e-06 in 243804 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.5191C>T has been reported in the literature in individuals affected with Hereditary Breast and Ovarian Cancer (Borg_2010, Azzollini_2016, Zanella_2017). One of these publications reported this variant in a proband who carried another pathogenic mutation, however the exact identity of this co-occurring variant was not specified (Azzollini_2016). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Three clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
not provided Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingQuest Diagnostics Nichols Institute San Juan CapistranoDec 07, 2022The frequency of this variant in the general population, 0.0000082 (2/243804 chromosomes, http://gnomad.broadinstitute.org), is uninformative in assessment of its pathogenicity. In the published literature, the variant has been reported in individuals and families affected with breast and/or ovarian cancer (PMIDs: 32438681 (2020), 27062684 (2016), 20104584 (2010)). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is benign or damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -
Hereditary breast ovarian cancer syndrome Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpNov 10, 2023This sequence change replaces histidine, which is basic and polar, with tyrosine, which is neutral and polar, at codon 1731 of the BRCA2 protein (p.His1731Tyr). This variant is present in population databases (rs80358745, gnomAD 0.002%). This missense change has been observed in individual(s) with breast cancer (PMID: 20104584, 27062684, 28263838). This variant is also known as 5419C>T. ClinVar contains an entry for this variant (Variation ID: 252409). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Breast-ovarian cancer, familial, susceptibility to, 2 Benign:1
Benign, criteria provided, single submitterclinical testingMyriad Genetics, Inc.Jun 23, 2023This variant is considered benign. This variant has been observed in trans with a known pathogenic variant in one or more individuals lacking clinical features consistent with gene-specific recessive disease. This variant is strongly associated with less severe personal and family histories of cancer, typical for individuals without pathogenic variants in this gene [PMID: 25085752]. -
