BRCA2 (HGNC:1101): (BRCA2 DNA repair associated) Inherited mutations in BRCA1 and this gene, BRCA2, confer increased lifetime risk of developing breast or ovarian cancer. Both BRCA1 and BRCA2 are involved in maintenance of genome stability, specifically the homologous recombination pathway for double-strand DNA repair. The largest exon in both genes is exon 11, which harbors the most important and frequent mutations in breast cancer patients. The BRCA2 gene was found on chromosome 13q12.3 in human. The BRCA2 protein contains several copies of a 70 aa motif called the BRC motif, and these motifs mediate binding to the RAD51 recombinase which functions in DNA repair. BRCA2 is considered a tumor suppressor gene, as tumors with BRCA2 mutations generally exhibit loss of heterozygosity (LOH) of the wild-type allele. [provided by RefSeq, May 2020]

The p.Q1998R variant (also known as c.5993A>G), located in coding exon 10 of the BRCA2 gene, results from an A to G substitution at nucleotide position 5993. The glutamine at codon 1998 is replaced by arginine, an amino acid with highly similar properties. This variant was reported in 0/60,466 breast cancer cases and in 1/53,461 controls (Dorling et al. N Engl J Med. 2021 02;384:428-439). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.

The p.Gln1998Arg variant was not identified in the literature. The variant was identified in dbSNP (ID: rs80358835 ) “With uncertain significance allele”, the ClinVar database 1X (classified as an uncertain significance variant), the BIC database (1X with unknown clinical importance), and UMD (1X as an unknown variant). The identification of this variant in an individual with a co-occuring pathogenic variant (BRCA2: p.Val2969CysfsX7) by our laboratory increased the likelihood the p.Gln1998Arg variant does not have clinical significance. The p.Gln1998 residue is not conserved in mammals and 4 out of 5 computational analyses (SIFT, AlignGVGD, BLOSUM, MutationTaster, Polyphen-2) do not suggest a high likelihood of impact to the protein. However, this information is not predictive enough to rule out pathogenicity. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of unknown significance.
