Missense variant in a coldspot region where missense variants are very unlikely to be pathogenic (PMID:31911673). -
This missense variant replaces proline with arginine at codon 2096 of the BRCA2 protein. Computational prediction suggests that this variant may not impact protein structure and function. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in three individuals affected with breast cancer, an individual affected with glioblastoma multiforme and in a suspected hereditary breast cancer family (PMID: 12955716, 21318380, 26689913, 32599251; Color internal data). A multifactorial analysis has reported a likelihood ratio for pathogenicity based on personal and family history of 0.301 from log(LR)=-0.522101402 for 3 carriers (PMID: 31853058). This variant has been identified in 1/31400 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.P2096R variant (also known as c.6287C>G), located in coding exon 10 of the BRCA2 gene, results from a C to G substitution at nucleotide position 6287. The proline at codon 2096 is replaced by arginine, an amino acid with dissimilar properties. This alteration was identified in multiple individuals diagnosed with breast cancer (Díez O et al. Hum. Mutat., 2003 Oct;22:301-12; Hansen TV et al. Fam Cancer. 2011; 10:207-12). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -

Variant summary: BRCA2 c.6287C>G (p.Pro2096Arg) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 244846 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.6287C>G has been reported in the literature in individuals affected with breast cancer (examples- Diez_2003, Hansen_2011, Petersen_2016, Pirim_2020). These reports do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. Co-occurrence with another pathogenic variant has been reported (BRCA1 c.427G>T, p.E143X; Petersen_2016), providing supporting evidence for a benign role. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Five clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -

In the published literature, this variant has been reported in individuals with breast cancer (PMID: 12955716 (2003), 21318380 (2011), 26733283 (2016), 32599251 (2020)), and glioblastoma (PMID: 26689913 (2015)). The frequency of this variant in the general population, 0.000032 (1/31400 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is benign or damaging. Additional analysis using software algorithms for the prediction of the effect of nucleotide changes on BRCA2 mRNA splicing yielded predictions that this variant may result in the gain of a cryptic splice site without affecting the natural splice sites . Based on the available information, we are unable to determine the clinical significance of this variant. -
