Variant summary
Our verdict is Likely benign. Variant got -1 ACMG points: 2P and 3B. PM2BP4_ModerateBP6
The NM_000059.4(BRCA2):c.6714T>G(p.Asp2238Glu) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000342 in 1,461,662 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 12/19 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another nucleotide change resulting in the same amino acid substitution has been previously reported as Uncertain significance in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. D2238G) has been classified as Likely benign.

Missense variant in a coldspot region where missense variants are very unlikely to be pathogenic (PMID:31911673). -
The p.D2238E variant (also known as c.6714T>G), located in coding exon 10 of the BRCA2 gene, results from a T to G substitution at nucleotide position 6714. The aspartic acid at codon 2238 is replaced by glutamic acid, an amino acid with highly similar properties. This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Observed in individuals with breast cancer (PMID: 33471991); Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant does not alter protein structure/function; Also known as 6942T>G; This variant is associated with the following publications: (PMID: 18724707, 27150160, 31131967, 33471991) -
The BRCA2 c.6714T>G; p.Asp2238Glu variant (rs28897742), to our knowledge, is not reported in the medical literature but is reported in ClinVar (Variation ID: 52167). This variant is only observed on one allele in the Genome Aggregation Database, indicating it is not a common polymorphism. Computational analyses are uncertain whether this variant is neutral or deleterious (REVEL: 0.437). Due to limited information, the clinical significance of this variant is uncertain at this time. -

The p.Asp2238Glu was not identified in the literature nor was it identified in HGMD, LOVD, or COSMIC. It was identified in the UMD one time and BIC two times, both with unknown significance, and the Clinvar database (1x from Invitae, classification not provided and 1x from BIC, as uncertain significance). It is listed in the dbSNP database (rs28897742) “With Uncertain significance allele”, but frequency information was not available from the general population (ESP project, 1000 genomes, HapMap). The p.Asp2238 variant is not well conserved in mammals and lower organisms and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) provide inconsistent predictions regarding the impact to the protein. However, this information is not very predictive of pathogenicity. In addition, in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) do not predict a difference in splicing. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of unknown significance. -
Hereditary breast ovarian cancer syndrome Uncertain:1
This sequence change replaces aspartic acid, which is acidic and polar, with glutamic acid, which is acidic and polar, at codon 2238 of the BRCA2 protein (p.Asp2238Glu). This variant is present in population databases (rs28897742, gnomAD 0.002%). This missense change has been observed in individual(s) with clinical features of BRCA2-related conditions (PMID: 10923033). ClinVar contains an entry for this variant (Variation ID: 52167). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
