The BRCA2 c.7682A>G; p.Gln2561Arg variant (rs55647716) is reported in the literature in individuals with pancreatic cancer (Blair 2018, Shindo 2017), and in an individual with head and neck squamous cell carcinoma (Huang 2018). This variant is also reported in ClinVar (Variation ID: 216258). It is found in the general population with an overall allele frequency of 0.002% (5/282686 alleles) in the Genome Aggregation Database. The glutamine is moderately conserved, and computational analyses are uncertain whether this variant is neutral or deleterious (REVEL: 0.435). This variant was found in 2/121276 control chromosomes at a frequency of 0.0000165, which does not exceed the estimated maximal expected allele frequency of a pathogenic BRCA2 variant (0.0007503). This variant has been reported in a head and neck squamous cell carcinoma sample as a germline variant without clear evidence supporting causality. In addition, multiple clinical diagnostic laboratories/reputable databases classified this variant as uncertain significance. Taken together, this variant is classified as VUS. -
In the published literature, this variant has been reported in individuals with pancreatic cancer and head and neck squamous cell carcinoma ((PMIDs: 26689913 (2015), 28767289 (2017), 29309945 (2018), 29625052 (2018)). This variant has also been reported to have a neutral effect on BRCA2 homology directed repair activity (PMID: 35736817 (2022)). The frequency of this variant in the general population, 0.0002 (5/24968 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -

Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). Splice site prediction tools suggest that this variant may not impact RNA splicing. To our knowledge, functional studies have not been performed for this variant. This variant has been reported in individuals affected with head and neck squamous cell carcinoma (PMID: 26689913) and pancreatic cancer (PMID: 28767289, 29309945). This variant has also been identified in 5/282686 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. Experimental studies have shown that this missense change does not substantially affect BRCA2 function (PMID: 35736817). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
