Variant summary: BRCA2 c.8699A>T (p.Asp2900Val) results in a non-conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251328 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.8699A>T has been reported in the literature as a VUS in settings of multigene panel testing among individuals with a family history or personal history of colorectal/breast cancer (example, Yurgelun_2017, Moradian_2021). These report(s) do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Six clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
The BRCA2 c.8699A>T; p.Asp2900Val variant (rs398122712), also reported as 8927A>T, has been described in an individual with colorectal cancer, but its association with disease was determined to be uncertain (Yurgelun 2017). It is reported as a variant of uncertain clinical significance in ClinVar (Variation ID: 91733), and is observed on only one allele in the Genome Aggregation Database, indicating it is not a common polymorphism. The aspartic acid at codon 2900 is highly conserved, and computational algorithms (PolyPhen-2, SIFT) predict that this variant is deleterious. However, due to limited or absent clinical and functional data regarding this variant, its clinical significance cannot be determined with certainty. REFERENCES Yurgelun M et al. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). A functional study has shown this variant does not impact sensitivity to PARP inhibitors (PMID: 32444794). This variant has been reported in individuals affected with breast cancer (PMID: 33471991, 33558524) and colorectal cancer (PMID: 28135145). This variant has been identified in 1/251328 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

This sequence change replaces aspartic acid, which is acidic and polar, with valine, which is neutral and non-polar, at codon 2900 of the BRCA2 protein (p.Asp2900Val). This variant is present in population databases (rs398122712, gnomAD no frequency). This missense change has been observed in individual(s) with breast cancer, colorectal cancer, and/or short telomeres (PMID: 28135145, 30995915, 33558524). ClinVar contains an entry for this variant (Variation ID: 91733). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. Experimental studies have shown that this missense change does not substantially affect BRCA2 function (PMID: 32444794). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
