The c.8940dupA pathogenic mutation, located in coding exon 21 of the BRCA2 gene, results from a duplication of A at nucleotide position 8940, causing a translational frameshift with a predicted alternate stop codon (p.E2981Rfs*37). This mutation has been identified in early onset breast cancer cohorts (Lee E et al. Breast Cancer Res. 2008 Mar;10:R19; Copson ER et al. Lancet Oncol. 2018 Feb;19(2):169-180). Of note, this variant has been reported as 9168insA in the literature. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -

The variant was absent in 245944 control chromosomes. c.8940dupA has been reported in the literature in individuals affected with Hereditary Breast And Ovarian Cancer Syndrome and subsequently cited by others (example, Rebbeck_2018, Michl_2016, Abida_2017, Park_2020). These data do not allow any conclusion about variant significance. To our knowledge, no variant specific experimental evidence demonstrating an impact on protein function has been reported. Four clinical diagnostic laboratories and one expert panel (ENIGMA) have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All submitters classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -

This duplication of one nucleotide in BRCA2 is denoted c.8940dupA at the cDNA level and p.Glu2981ArgfsX37 (E2981RfsX37) at the protein level. The normal sequence, with the base that is duplicated in braces, is CAAAAAA[A]GAAA. The duplication causes a frameshift which changes a Glutamic Acid to an Arginine at codon 2981, and creates a premature stop codon at position 37 of the new reading frame. This variant is predicted to cause loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay. BRCA2 c.8940dupA, also reported as 9168insA, has been reported in at least one individual with early-onset breast cancer (Lee 2008). We consider this variant to be pathogenic. -
