Uncertain significance, criteria provided, single submitter | clinical testing | Quest Diagnostics Nichols Institute San Juan Capistrano | May 31, 2023 | In the published literature, the variant has been reported in individuals with breast cancer (PMID: 22034289 (2012), 35264596 (2022)). The frequency of this variant in the general population, 0.000008 (2/249930 chromosomes, http://gnomad.broadinstitute.org), is uninformative in assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. - |

Uncertain significance, criteria provided, single submitter | clinical testing | Ambry Genetics | Oct 13, 2023 | The p.Y3226C variant (also known as c.9677A>G), located in coding exon 26 of the BRCA2 gene, results from an A to G substitution at nucleotide position 9677. The tyrosine at codon 3226 is replaced by cysteine, an amino acid with highly dissimilar properties. This alteration has been identified in multiple individuals diagnosed with breast cancer (Fackenthal JD et al. Int. J. Cancer. 2012 Sep;13:1114-23; Guindalini RSC et al. Sci Rep, 2022 Mar;12:4190). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |

Uncertain significance, criteria provided, single submitter | clinical testing | Women's Health and Genetics/Laboratory Corporation of America, LabCorp | May 16, 2022 | Variant summary: Variant summary: BRCA2 c.9677A>G (p.Tyr3226Cys) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 8e-06 in 249930 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.9677A>G has been reported in the literature in an individual affected with Breast Cancer Syndrome (example, Fackenthal_2012). This report(s) does not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. At-least one co-occurrences with another pathogenic variant has been reported in the UMD database (BRCA2 c.7501C>T, p.Gln2501*), providing supporting evidence for a benign role. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Eight clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation 7 classified as VUS while one classified as likely benign. Based on the evidence outlined above, the variant was classified as VUS-possibly benign. - |
BRCA2-related disorder Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | PreventionGenetics, part of Exact Sciences | Feb 16, 2023 | The BRCA2 c.9677A>G variant is predicted to result in the amino acid substitution p.Tyr3226Cys. This variant has been reported in one patient with breast cancer (Fackenthal et al. 2012. PubMed ID: 22034289). This variant is reported in 0.012% of alleles in individuals of African descent in gnomAD (http://gnomad.broadinstitute.org/variant/13-32972327-A-G). This variant is classified as likely benign and variant of uncertain significance in Clinvar (https://www.ncbi.nlm.nih.gov/clinvar/variation/52890). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. - |

Hereditary breast ovarian cancer syndrome Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Jan 09, 2024 | This sequence change replaces tyrosine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 3226 of the BRCA2 protein (p.Tyr3226Cys). This variant is present in population databases (rs80359237, gnomAD 0.01%). This missense change has been observed in individual(s) with breast cancer (PMID: 22034289, 35264596). ClinVar contains an entry for this variant (Variation ID: 52890). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. - |
