Variant summary: The BRCA2 c.9730G>T (p.Val3244Phe) variant involves the alteration of a non-conserved nucleotide. 3/4 in silico tools predict a benign outcome for this variant (SNPs&GO not captured due to low reliability index). This variant was found in 1/121354 control chromosomes at a frequency of 0.0000082, which does not exceed the estimated maximal expected allele frequency of a pathogenic BRCA2 variant (0.0007503). In addition, multiple clinical diagnostic laboratories/reputable databases classified this variant as uncertain significance/likely neutral. The variant of interest has not, to our knowledge, been reported in affected individuals via publications and/or reputable databases/clinical diagnostic laboratories; nor evaluated for functional impact by in vivo/vitro studies. Because of the absence of clinical information and the lack of functional studies, the variant is classified as a variant of uncertain significance (VUS) until additional information becomes available.
The frequency of this variant in the general population, 0.000012 (3/251200 chromosomes, http://gnomad.broadinstitute.org), is uninformative in assessment of its pathogenicity. In the published literature, the variant has been reported in an individual affected with breast cancer (PMID: 28664449 (2017)). It also has been reported in unaffected individuals in a large breast cancer screening study (PMID: 32467295 (2020)). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant.
This variant is denoted BRCA2 c.9730G>T at the cDNA level, p.Val3244Phe (V3244F) at the protein level, and results in the change of a Valine to a Phenylalanine (GTC>TTC). Using alternate nomenclature, this variant would be defined as BRCA2 9958G>T. This variant has been observed in at least one individual with breast cancer (Li 2017). BRCA2 Val3244Phe was not observed at a significant allele frequency in large population cohorts (NHLBI Exome Sequencing Project, The 1000 Genomes Consortium 2015, Lek 2016). Since Valine and Phenylalanine differ in some properties, this is considered a semi-conservative amino acid substitution. BRCA2 Val3244Phe occurs at a position that is not conserved and is not located in a known functional domain. In silico analyses predict that this variant is unlikely to alter protein structure or function. Based on currently available evidence, it is unclear whether BRCA2 Val3244Phe is a pathogenic or benign variant. We consider it to be a variant of uncertain significance.

The p.V3244F variant (also known as c.9730G>T), located in coding exon 26 of the BRCA2 gene, results from a G to T substitution at nucleotide position 9730. The valine at codon 3244 is replaced by phenylalanine, an amino acid with highly similar properties. This alteration has been reported in 1/313 unselected individuals diagnosed with breast cancer (Li G et al. J. Cancer Res. Clin. Oncol., 2017 Oct;143:2011-2024). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear.
Hereditary breast ovarian cancer syndrome Uncertain:1
This sequence change replaces valine, which is neutral and non-polar, with phenylalanine, which is neutral and non-polar, at codon 3244 of the BRCA2 protein (p.Val3244Phe). This variant is present in population databases (rs11571831, gnomAD 0.01%). This missense change has been observed in individual(s) with breast and/or ovarian cancer (PMID: 28664449). ClinVar contains an entry for this variant (Variation ID: 38263). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (internal data). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
