These proteins are components of the heterotetrameric adaptor protein complexes, which play important roles in the secretory and endocytic pathways by mediating vesicle formation and sorting of integral membrane proteins. The encoded protein is the small subunit of adaptor protein complex-4, which is associated with both clathrin- and nonclathrin-coated vesicles. Mutations in this gene are associated with spastic quadriplegic cerebral palsy-6. Alternatively spliced transcript variants encoding multiple isoforms have been observed for this gene, and a pseudogene of this gene is located on the long arm of chromosome 6. This nonsense variant found in exon 4 of 6 is predicted to result in loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay. Loss-of-function variation in AP4S1 is an established mechanism of disease (PMID: 21620353, 25552650). This variant has been previously reported as a compound heterozygous and homozygous change in individuals with spastic paraplegia (PMID: 25552650, 28708303, 31130284, 32979048, 32371413). The c.289C>T (p.Arg97Ter) variant is present in the heterozygous state in the gnomAD population database at a frequency of 0.01% (31/282750). Analysis of the parental samples showed the mother is heterozygous and the father is heterozygous for this variant. Based on the available evidence, c.289C>T (p.Arg97Ter) is classified as Pathogenic. -
