The p.C40Y variant (also known as c.119G>A), located in coding exon 1 of the MLH3 gene, results from a G to A substitution at nucleotide position 119. The cysteine at codon 40 is replaced by tyrosine, an amino acid with highly dissimilar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. The evidence for this gene-disease relationship is limited; therefore, the clinical significance of this alteration is unclear. -

This sequence change replaces cysteine, which is neutral and slightly polar, with tyrosine, which is neutral and polar, at codon 40 of the MLH3 protein (p.Cys40Tyr). This variant is present in population databases (rs746448677, gnomAD 0.08%), and has an allele count higher than expected for a pathogenic variant. This missense change has been observed in individual(s) with diffuse large B cell lymphoma (PMID: 23960188). ClinVar contains an entry for this variant (Variation ID: 410903). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive.
