Genes affected
CHST14 (HGNC:24464): (carbohydrate sulfotransferase 14) This gene encodes a member of the HNK-1 family of sulfotransferases. The encoded protein transfers sulfate to the C-4 hydroxyl of N-acetylgalactosamine residues in dermatan sulfate. Mutations in this gene have been associated with adducted thumb-clubfoot syndrome.[provided by RefSeq, Mar 2010]

ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 8 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most exon, not predicted to undergo nonsense mediated mRNA decay. Fraction of 0.953 CDS is truncated, and there are 2 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 15-40471264-CCTGGG-C is Pathogenic according to our data. Variant chr15-40471264-CCTGGG-C is described in ClinVar as [Pathogenic]. Clinvar id is 3653043.Status of the report is criteria_provided_single_submitter, 1 stars.

This sequence change creates a premature translational stop signal (p.Leu18Profs*126) in the CHST14 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 359 amino acid(s) of the CHST14 protein. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with CHST14-related conditions. This variant disrupts a region of the CHST14 protein in which other variant(s) (p.Tyr293Cys) have been determined to be pathogenic (PMID: 10766984, 20004762, 20533528, 21744491). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. -
