This sequence change replaces histidine, which is basic and polar, with leucine, which is neutral and non-polar, at codon 334 of the CAPN3 protein (p.His334Leu). This variant is present in population databases (rs749863676, gnomAD 0.01%). This missense change has been observed in individual(s) with autosomal recessive CAPN3-related conditions (PMID: 18055493, 30564623; internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 285580). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt CAPN3 protein function with a positive predictive value of 80%. This variant disrupts the p.His334 amino acid residue in CAPN3. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 9150160). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. Variant summary: CAPN3 c.1001A>T (p.His334Leu) results in a non-conservative amino acid change located in the Peptidase C2, calpain, catalytic domain (IPR001300) of the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant allele was found at a frequency of 4e-06 in 251216 control chromosomes. c.1001A>T has been observed in individual(s) affected with Limb-Girdle Muscular Dystrophy, Autosomal Recessive (example: Groen_2007, Nallamilli_2018, Internal data) . These data indicate that the variant is likely to be associated with disease. The following publications have been ascertained in the context of this evaluation (PMID: 18055493, 30564623). ClinVar contains an entry for this variant (Variation ID: 285580). Based on the evidence outlined above, the variant was classified as likely pathogenic.

The c.1001A>T (p.H334L) alteration is located in exon 7 (coding exon 7) of the CAPN3 gene. This alteration results from an A to T substitution at nucleotide position 1001, causing the histidine (H) at amino acid position 334 to be replaced by a leucine (L). for autosomal recessive limb-girdle muscular dystrophy; however, its clinical significance for autosomal dominant limb-girdle muscular dystrophy is uncertain. Based on data from gnomAD, the T allele has an overall frequency of 0.001% (3/282620) total alleles studied. The highest observed frequency was 0.012% (3/24960) of African alleles. This variant has been identified in the homozygous state and/or in conjunction with other CAPN3 variant(s) in individual(s) with features consistent with autosomal recessive limb-girdle muscular dystrophy; in at least one instance, the variants were identified in trans (Nallamilli, 2018; external communication). This amino acid position is highly conserved in available vertebrate species. Based on internal structural analysis, this variant is anticipated to disrupt a region of known function (Ye, 2018; Ambry internal data). This alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, this alteration is classified as pathogenic.
