Variant summary
Our verdict is Uncertain significance. Variant got 4 ACMG points: 4P and 0B. PM2PP3_Moderate
The NM_001145102.2(SMAD3):c.-78C>T variant causes a 5 prime UTR premature start codon gain change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★).

Familial thoracic aortic aneurysm and aortic dissection Uncertain:2
This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 80 of the SMAD3 protein (p.Arg80Trp). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with aortic dilation (PMID: 33059708). ClinVar contains an entry for this variant (Variation ID: 405563). Advanced modeling performed at Invitae incorporating data from internal and/or published experimental studies (Invitae) indicates that this missense variant is not expected to disrupt SMAD3 function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
The p.R80W variant (also known as c.238C>T), located in coding exon 2 of the SMAD3 gene, results from a C to T substitution at nucleotide position 238. The arginine at codon 80 is replaced by tryptophan, an amino acid with dissimilar properties. This alteration has been reported in a Marfan syndrome genetic testing cohort (Stengl R et al. Orphanet J Rare Dis, 2020 Oct;15:290). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
Aneurysm-osteoarthritis syndrome Uncertain:2

This missense variant replaces arginine with tryptophan at codon 80 of the SMAD3 protein. Computational prediction suggests that this variant may have a deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with SMAD3-related disorders in the literature. This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
