POLG (HGNC:9179): (DNA polymerase gamma, catalytic subunit) Mitochondrial DNA polymerase is heterotrimeric, consisting of a homodimer of accessory subunits plus a catalytic subunit. The protein encoded by this gene is the catalytic subunit of mitochondrial DNA polymerase. The encoded protein contains a polyglutamine tract near its N-terminus that may be polymorphic. Defects in this gene are a cause of progressive external ophthalmoplegia with mitochondrial DNA deletions 1 (PEOA1), sensory ataxic neuropathy dysarthria and ophthalmoparesis (SANDO), Alpers-Huttenlocher syndrome (AHS), and mitochondrial neurogastrointestinal encephalopathy syndrome (MNGIE). Likely pathogenic, criteria provided, single submitter | clinical testing | GeneDx | Sep 28, 2012 | p.Met1163Thr (ATG>ACG):c.3488 T>C in exon 22 of the POLG gene (NM_002693.2). The Met1163Thr missense change has not been published as a mutation, nor has it been reported as a benign polymorphism to our knowledge. The NHLBI ESP Exome Variant Project has not identified Met1163Thr in approximately 6,500 individuals of European or African American ethnicity, indicating that it is not a common benign variant in these populations. The amino acid substitution is non-conservative as a non-polar Methionine residue is replaced by a polar Threonine residue. It alters a conserved position in the protein and multiple in silico algorithms predict that Met1163Thr may be damaging to the structure/function of the protein. A different missense substitution at the same codon (Met1163Arg) was identified in a patient with progressive encephalopathy (Kolberg et al., 2006) and other missense mutations at neighboring codons in the polymerase domain of the protein (N1157S, S1176L) have been reported in association with POLG-related disorders. Therefore, based on the currently available information, Met1163Thr is a strong candidate for a disease-causing mutation, although the possibility that it is a benign variant cannot be excluded.
