Variant summary
Our verdict is Pathogenic. The variant received 10 ACMG points: 10P and 0B. PM1PM2PM5PP3_ModeratePP5_Moderate
The NM_002693.3(POLG):c.1874C>G(p.Pro625Arg) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. P625L) has been classified as Likely pathogenic.

Progressive sclerosing poliodystrophy Pathogenic:1
This sequence change replaces proline, which is neutral and non-polar, with arginine, which is basic and polar, at codon 625 of the POLG protein (p.Pro625Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with Alpers-Huttenlocher syndrome (PMID: 20883824). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt POLG protein function. Experimental studies have shown that this missense change affects POLG function (PMID: 20883824). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -
