This missense variant replaces aspartic acid with asparagine at codon 140 of the MYH11 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with osteogenesis imperfecta (PMID: 30684648). The individual's sister, niece and father were also diagnosed with osteogenesis imperfecta, and father was also affected with aortic dissection. This variant has been identified in 3/282890 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.D140N variant (also known as c.418G>A), located in coding exon 2 of the MYH11 gene, results from a G to A substitution at nucleotide position 418. The aspartic acid at codon 140 is replaced by asparagine, an amino acid with highly similar properties. This variant has been reported in a subject with features of osteogenesis imperfecta and a family history of thoracic aortic aneurysm and dissection, who also carried a frameshift alteration in COL1A1 (Balasubramanian M et al. Bone, 2019 Apr;121:191-195). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -

A variant of uncertain significance has been identified in the MYH11 gene. The D140N variant has not been published as pathogenic or been reported as benign to our knowledge. This variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The D140N variant is a semi-conservative amino acid substitution, which may impact secondary protein structure as these residues differ in some properties. However, this substitution occurs at a position where amino acids with similar properties to aspartic acid are tolerated across species. Finally, in silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. -
