This missense variant replaces tryptophan with arginine at codon 167 of the TSC2 protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). Functional studies have shown that this variant retained >80% of wild-type TSC2 (Almeida 2019, Dissertation, University of Sao Paulo; LOVD). This variant has been reported in a non-tuberous sclerosis patient affected with gastroenteropancreatic neuroendocrine neoplasias (PMID: 29167182). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The p.W167R variant (also known as c.499T>C), located in coding exon 5 of the TSC2 gene, results from a T to C substitution at nucleotide position 499. The tryptophan at codon 167 is replaced by arginine, an amino acid with dissimilar properties. This alteration has been reported in a non-syndromic gastroenteropancreatic neuroendocrine neoplasia patient (Asprino PF et al. Endocr. Relat. Cancer, 2018 Feb;25:L1-L5). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
