This TSC2 missense variant (rs45514100) is rare (<0.1%) in a large population dataset (gnomAD v4.1.0: 3/1612924 total alleles; 0.0002%; no homozygotes). It has been reported in ClinVar (Variation ID 238007), but has not been reported in the literature, to our knowledge. Of two bioinformatics tools queried, one predicts that the substitution would be damaging, while the other predicts that it would be tolerated, and the glutamine residue at this position is evolutionarily conserved in all but one of the species assessed. We consider the clinical significance of c.3028C>G in TSC2 to be uncertain at this time.

This missense variant replaces glutamine with glutamic acid at codon 1010 of the TSC2 protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with tuberous sclerosis complex in the literature. This variant has been identified in 2/281892 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

The p.Q1010E variant (also known as c.3028C>G), located in coding exon 26 of the TSC2 gene, results from a C to G substitution at nucleotide position 3028. The glutamine at codon 1010 is replaced by glutamic acid, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
