A TSC2 c.3365G>C (p.Arg1122Pro) variant was identified at a near heterozygous allelic fraction of 47.9%, a frequency which may be consistent with germline origin. This variant, to our knowledge, has not been reported in the medical literature but has been reported in the ClinVar database as a germline variant of uncertain significance by four submitters (ClinVar Variation ID: 451736). It is only observed on 5/1,607,174 alleles in the general population (gnomAD v.4.1.0), indicating it is not a common variant. Computational predictors are uncertain as to the impact of this variant on TSC2 function. Due to limited information, and based on ACMG/AMP guidelines for variant interpretation (Richards S et al., PMID: 25741868), the clinical significance of this variant is uncertain at this time.

A variant of uncertain significance has been identified in the TSC2 gene. The R1122P variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. The R1122P variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The R1122P variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. However, this substitution occurs at a position that is not conserved. Additionally, this substitution does not occur within known functional domains of the tuberin protein, where many pathogenic missense variants have been identified (Northrup et al., 2011; Au et al., 2007). In silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic variant or a rare benign variant.

The p.R1122P variant (also known as c.3365G>C), located in coding exon 28 of the TSC2 gene, results from a G to C substitution at nucleotide position 3365. The arginine at codon 1122 is replaced by proline, an amino acid with dissimilar properties. This variant was detected as heterozygous in individuals with no reported features of tuberous sclerosis complex (Ambry internal data). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear.
