This sequence change replaces serine, which is neutral and polar, with tryptophan, which is neutral and slightly polar, at codon 1433 of the TSC2 protein (p.Ser1433Trp). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of tuberous sclerosis complex (PMID: 32917966). ClinVar contains an entry for this variant (Variation ID: 1302975). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt TSC2 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

The p.S1433W variant (also known as c.4298C>G), located in coding exon 33 of the TSC2 gene, results from a C to G substitution at nucleotide position 4298. The serine at codon 1433 is replaced by tryptophan, an amino acid with highly dissimilar properties. This alteration was identified in 1 of 374 patients with clinically suspected TSC undergoing genetic testing within the TSC1 and TSC2 genes (Meng Y et al. J Hum Genet, 2021 Mar;66:227-236). This amino acid position is poorly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.
