Familial cancer of breast Uncertain:2Benign:1
Curators: Marc Tischkowitz, Arleen D. Auerbach. Submitter to LOVD: Marc Tischkowitz.
This sequence change replaces histidine, which is basic and polar, with arginine, which is basic and polar, at codon 1158 of the PALB2 protein (p.His1158Arg). This variant is present in population databases (rs45505500, gnomAD 0.004%). This missense change has been observed in individual(s) with breast cancer (PMID: 17200668). ClinVar contains an entry for this variant (Variation ID: 216756). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
This variant is considered likely benign. This variant is strongly associated with less severe personal and family histories of cancer, typical for individuals without pathogenic variants in this gene [PMID: 25085752].
not provided Uncertain:2
The PALB2 c.3473A>G (p.His1158Arg) variant has been reported in the published literature in individuals with breast cancer (PMIDs: 25186627 (2015), 17200668 (2007)), as well as in a reportedly healthy individual (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared/genes/PALB2)). The frequency of this variant in the general population, 0.000023 (3/129164 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant.
Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis indicates that this missense variant does not alter protein structure/function; This variant is associated with the following publications: (PMID: 24485656, 19609323, 20871615, 17200668, 33471991, 25186627)
Hereditary cancer-predisposing syndrome Uncertain:1Benign:1
This missense variant replaces histidine with arginine at codon 1158 of the PALB2 protein. Computational prediction suggests that this variant may not impact protein structure and function. To our knowledge, functional studies have not been performed for this variant. This variant has been reported in two individuals affected with breast cancer (PMID: 17200668, 25186627) and in a breast cancer case-control meta-analysis in 0/60466 cases and 2/53461 unaffected individuals (PMID: 33471991; Leiden Open Variation Database DB-ID PALB2_010287). This variant has also been identified in 4/282838 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity.
PALB2-related disorder Uncertain:1
The PALB2 c.3473A>G variant is predicted to result in the amino acid substitution p.His1158Arg. This variant has been reported in both controls and individuals with breast cancer (Rahman et al. 2007. PubMed ID: 17200668; Tung et al. 2015. PubMed ID: 25186627; Dorling et al. 2021. PubMed ID: 33471991). This variant is reported in 0.0040% of alleles in individuals of African descent in gnomAD. This variant is interpreted as a variant of uncertain significance in ClinVar (https://www.ncbi.nlm.nih.gov/clinvar/variation/216756/). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.
