Variant summary
Our verdict is Likely benign. The variant received -5 ACMG points: 0P and 5B. BP4_StrongBP6
The NM_024675.4(PALB2):c.2329G>A(p.Asp777Asn) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000217 in 1,614,078 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 16/22 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. D777Y) has been classified as Likely benign.

This variant is classified as a variant of uncertain significance as there is insufficient evidence to determine its impact on protein function and/or cancer risk. -
Hereditary cancer-predisposing syndrome Uncertain:1Benign:2
- -
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -
BP1 c.2329G>A, located in exon 5 of the PALB2 gene, is predicted to result in the substitution of aspartic acid by asparagine at codon 777, p.(Asp777Asn). This position is outside a (potentially) clinically important functional domain and, moreover, the SpliceAI algorithm predicts no significant impact on splicing (BP1).This variant is found in 4/268309 alleles at a frequency of 0.001% in the gnomAD v2.1.1 database, non-cancer dataset. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. This variant has been reported in cancer-affected and unaffected individuals in case-control studies (PMID:33471991, 25225577, 30309218). This variant has been reported in the ClinVar database (3x likely benign, 4x uncertain significance), and in the LOVD (2x benign, 1x uncertain significance). Based on currently available information, the variant c.2329G>A should be considered an uncertain significance variant, according to ClinGen Hereditary Breast, Ovarian and Pancreatic Cancer Expert Panel Specifications to the ACMG/AMP Variant Interpretation Guidelines for PALB2 Version 1.0.0. -
not provided Uncertain:2
Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant does not alter protein structure/function; Observed in individuals with breast and/or ovarian cancer but also in unaffected controls (PMID: 25225577, 28779002, 30309218, 33471991); This variant is associated with the following publications: (PMID: 24293293, 22895193, 25225577, 26976419, 30309218, 28779002, 31822803, 36922933, 24485656, 33471991) -
The frequency of this variant in the general population, 0.000016 (4/251432 chromosomes, http://gnomad.broadinstitute.org), is uninformative in assessment of its pathogenicity. In the published literature, the variant has been reported in affected individuals with breast cancer as well as in control individuals in a large scale breast cancer association study (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared/genes/PALB2)). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -
