This sequence change replaces aspartic acid, which is acidic and polar, with glycine, which is neutral and non-polar, at codon 616 of the PALB2 protein (p.Asp616Gly). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with breast cancer (PMID: 25186627). ClinVar contains an entry for this variant (Variation ID: 232905). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PALB2 protein function with a positive predictive value of 80%. Experimental studies have shown that this missense change does not substantially affect PALB2 function (PMID: 31636395). RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (Invitae). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.D616G variant (also known as c.1847A>G), located in coding exon 5 of the PALB2 gene, results from an A to G substitution at nucleotide position 1847. The aspartic acid at codon 616 is replaced by glycine, an amino acid with similar properties. This alteration has been reported in a Caucasian female who was diagnosed with breast cancer at age 43 (Tung N et al. Cancer, 2015 Jan;121:25-33). In a functional study, analyzing the impact of multiple PALB2 missense variants on homology directed DNA repair (HDR), p.D616G demonstrated HDR activity similar to wildtype (Wiltshire T et al. Genet. Med., 2019 Oct).This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -

Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). A functional study has shown that this variant slightly reduces homology directed repair activity of PALB2 to 88% of the wild-type protein (PMID: 31636395). This variant has been reported in one individual affected with breast cancer in the literature (PMID: 25186627). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
