This variant is denoted PALB2 c.1232G>A at the cDNA level, p.Arg411Lys (R411K) at the protein level, and results in the change of an Arginine to a Lysine (AGA>AAA). This variant has not, to our knowledge, been published in the literature as pathogenic or benign. PALB2 Arg411Lys was not observed in large population cohorts (NHLBI Exome Sequencing Project, The 1000 Genomes Consortium 2015, Lek 2016). Since Arginine and Lysine share similar properties, this is considered a conservative amino acid substitution. PALB2 Arg411Lys occurs at a position that is conserved across species and is located within the DNA binding domain and the Chromatin-association motif (ChAM) (UniProt). In silico analyses are inconsistent regarding the effect this variant may have on protein structure and function. Based on currently available evidence, it is unclear whether PALB2 Arg411Lys is a pathogenic or benign variant. We consider it to be a variant of uncertain significance. -

This missense variant replaces arginine with lysine at codon 411 of the PALB2 protein. Computational prediction suggests that this variant may not impact protein structure and function. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in a breast cancer and a pancreatic cancer case-control study, in which this variant was absent in 7051 breast cancer cases and 1005 pancreatic cancer cases, but it was detected in 1/11241 and 1/23705 unaffected individuals, respectively (PMID: 30287823, 32980694). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The p.R411K variant (also known as c.1232G>A), located in coding exon 4 of the PALB2 gene, results from a G to A substitution at nucleotide position 1232. The arginine at codon 411 is replaced by lysine, an amino acid with highly similar properties. This alteration was not observed in 7,051 unselected female breast cancer patients and was observed with an allele frequency of 0.00009 in 11,241 female controls of Japanese ancestry (Momozawa Y et al. Nat Commun, 2018 Oct;9:4083). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with PALB2-related conditions. This missense change has been observed to co-occur in individuals with a different variant in PALB2 that has been determined to be pathogenic (internal data), but the significance of this finding is unclear. ClinVar contains an entry for this variant (Variation ID: 449868). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt PALB2 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
