This variant is considered likely benign. This variant is strongly associated with less severe personal and family histories of cancer, typical for individuals without pathogenic variants in this gene [PMID: 25085752].
This sequence change replaces proline, which is neutral and non-polar, with alanine, which is neutral and non-polar, at codon 405 of the PALB2 protein (p.Pro405Ala). This variant is present in population databases (rs545119348, gnomAD 0.03%). This missense change has been observed in individual(s) with breast cancer and/or colorectal cancer (PMID: 27978560, 28825143, 33309985, 33811135). ClinVar contains an entry for this variant (Variation ID: 410151). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt PALB2 protein function with a positive predictive value of 80%. Experimental studies have shown that this missense change does not substantially affect PALB2 function (PMID: 33811135, 35853885). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

This missense variant replaces proline with alanine at codon 405 of the PALB2 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual with breast cancer (PMID: 28825143) and in an individual with colon cancer (PMID: 27978560). In large cancer case-control studies, this variant has not shown a significant association with breast cancer (PMID: 33471991) or colorectal cancer (PMID: 33309985). This variant has been identified in 7/282248 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity.

This variant has been reported in an individual with breast cancer, but has also been reported in controls from a breast cancer cohort study and a colorectal cancer cohort study (Zhang et al. 2017. PubMed ID: 28825143; Table S1, Momozama et al. 2018. PubMed ID: 30287823; Table S5, Fujita et al. 2020. PubMed ID: 33309985). In vitro experimental studies suggest this variant does not impact PAMB2 chromatin association (Bleuyard et al. 2017. PubMed ID: 29387807). This variant is reported in 0.035% of alleles in individuals of East Asian descent in gnomAD (http://gnomad.broadinstitute.org/variant/16-23646654-G-C) and has conflicting interpretations of likely benign and uncertain significance (https://preview.ncbi.nlm.nih.gov/clinvar/variation/410151/). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.
