This missense variant replaces serine with tyrosine at codon 319 of the PALB2 protein. Computational prediction suggests that this variant may not impact protein structure and function. Functional studies reported that this variant did not impact PALB2 function in homology-directed repair, DNA damage localization and PARP inhibitor sensitivity assays (PMID: 31586400, 31636395) and partially reduced binding to BRCA1 and RAD51 foci formation (PMID: 31586400). This variant has been reported in breast cancer case-control studies in 1 case and 2 unaffected individuals (PMID: 30287823, 33471991; Leiden Open Variation Database DB-ID PALB2_010187) and in a hereditary breast cancer family (PMID: 23448497). This variant also has been reported in pancreatic and prostate cancer case-control studies in which it was not detected in cases and observed in 3 unaffected individuals (PMID: 31214711, 32980694). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
The p.S319Y variant (also known as c.956C>A), located in coding exon 4 of the PALB2 gene, results from a C to A substitution at nucleotide position 956. The serine at codon 319 is replaced by tyrosine, an amino acid with dissimilar properties. This alteration was reported in 1/747 Australasian women with a personal history of early-onset breast cancer and a family history of multiple cases of breast cancer (Teo ZL et al. Breast Cancer Res. 2013 Feb; 15(1):R17). However, this alteration was also observed with an allele frequency of 0.0009 in 11241 female controls of Japanese ancestry and was not observed in 7051 unselected breast cancer cases (Momozawa Y et al Nat Commun 2018 Oct;9(1):4083). In one study, this alteration was found to be functionally normal in a homology-directed DNA repair (HDR) assay (Wiltshire T et al. Genet Med, 2020 03;22:622-632). In another study, this alteration was found to have normal activity in a PARP inhibitor sensitivity assay, a RAD51 foci formation assay and a homology-directed DNA repair (HDR) assay, but was found to have abnormal activity in a BRCA1/BRCA2 binding assay (Rodrigue A et al. Nucleic Acids Res, 2019 11;47:10662-10677). This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
Familial cancer of breast Uncertain:2
This sequence change replaces serine, which is neutral and polar, with tyrosine, which is neutral and polar, at codon 319 of the PALB2 protein (p.Ser319Tyr). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with breast cancer (PMID: 23448497). ClinVar contains an entry for this variant (Variation ID: 126782). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt PALB2 protein function with a negative predictive value of 80%. Experimental studies are conflicting or provide insufficient evidence to determine the effect of this variant on PALB2 function (PMID: 31586400, 31636395, 33139182). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
