This sequence change replaces proline, which is neutral and non-polar, with arginine, which is basic and polar, at codon 207 of the PALB2 protein (p.Pro207Arg). This variant is present in population databases (no rsID available, gnomAD 0.003%). This missense change has been observed in individual(s) with breast cancer (PMID: 22052327). ClinVar contains an entry for this variant (Variation ID: 126760). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change does not substantially affect PALB2 function (PMID: 31586400). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The frequency of this variant in the general population, 0.000004 (1/251410 chromosomes, http://gnomad.broadinstitute.org), is uninformative in assessment of its pathogenicity. In the published literature, the variant has been reported in an individual with personal and/or family history of breast and/or ovarian cancer, including male breast cancer (PMID: 22052327 (2012)). An experimental study reports this variant does not significantly affect PALB2 protein function, however further studies are needed to determine an overall impact (PMID: 31586400 (2019)). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is benign or damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -

Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in a family affected with breast and/or ovarian cancer (PMID: 22052327) and in an unaffected control individual in a breast cancer case-control study (PMID: 33471991). This variant has been identified in 1/251410 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
