Hereditary diffuse gastric adenocarcinoma Uncertain:2
This sequence change replaces isoleucine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 225 of the CDH1 protein (p.Ile225Thr). This variant is present in population databases (rs786203207, gnomAD 0.002%). This missense change has been observed in individual(s) with personal and/or family history of breast cancer, ovarian cancer, or pancreatic cancer (PMID: 32957588, 34299313, 36436516). ClinVar contains an entry for this variant (Variation ID: 186769). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

Hereditary cancer-predisposing syndrome Uncertain:2
The p.I225T variant (also known as c.674T>C), located in coding exon 5 of the CDH1 gene, results from a T to C substitution at nucleotide position 674. The isoleucine at codon 225 is replaced by threonine, an amino acid with similar properties. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.
This missense variant replaces isoleucine with threonine at codon 225 of the CDH1 protein. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in individuals with a personal and/or family history of breast, ovarian, or pancreatic cancer (PMID: 32957588, 36436516). This variant has been identified in 1/250900 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

Variant summary: CDH1 c.674T>C (p.Ile225Thr) results in a non-conservative amino acid change located in the Cadherin-like domain (IPR002126) of the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 250900 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.674T>C has been reported in the literature as a VUS in settings of multigene panel testing among individuals with infiltrating ductal carcinoma (IDC)/breast cancer (example, Germani_2020, Guglielmi_2021). These report(s) do not provide unequivocal conclusions about association of the variant with CDH1-related Hereditary Diffuse Gastric Cancer/Lobular Breast Cancer. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Three clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance.
