The p.I225T variant (also known as c.674T>C), located in coding exon 5 of the CDH1 gene, results from a T to C substitution at nucleotide position 674. The isoleucine at codon 225 is replaced by threonine, an amino acid with similar properties. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 250900 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.674T>C has been reported in the literature as a VUS in settings of multigene panel testing among individuals with infiltrating ductal carcinoma (IDC)/breast cancer (example, Germani_2020, Guglielmi_2021). These report(s) do not provide unequivocal conclusions about association of the variant with CDH1-related Hereditary Diffuse Gastric Cancer/Lobular Breast Cancer. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Three clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation.
