Genes affected
CDH1 (HGNC:1748): (cadherin 1) This gene encodes a classical cadherin of the cadherin superfamily. Alternative splicing results in multiple transcript variants, at least one of which encodes a preproprotein that is proteolytically processed to generate the mature glycoprotein. This calcium-dependent cell-cell adhesion protein is comprised of five extracellular cadherin repeats, a transmembrane region and a highly conserved cytoplasmic tail. Mutations in this gene are correlated with gastric, breast, colorectal, thyroid and ovarian cancer. Loss of function of this gene is thought to contribute to cancer progression by increasing proliferation, invasion, and/or metastasis. The ectodomain of this protein mediates bacterial adhesion to mammalian cells and the cytoplasmic domain is required for internalization. This gene is present in a gene cluster with other members of the cadherin family on chromosome 16. [provided by RefSeq, Nov 2015]

This missense variant replaces aspartic acid with glycine at codon 750 of the CDH1 protein. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in individuals affected with colorectal cancer (PMID: 28135145) and breast and/or ovarian cancer (PMID: 29470806). This variant has been identified in 10/251486 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

The p.D750G variant (also known as c.2249A>G), located in coding exon 14 of the CDH1 gene, results from an A to G substitution at nucleotide position 2249. The aspartic acid at codon 750 is replaced by glycine, an amino acid with similar properties. This variant has been reported in an individual affected with colorectal cancer (Yurgelun MB et al. J Clin Oncol, 2017 Apr;35:1086-1095). This variant was also observed in a study of 1010 unrelated Indian patients with breast and/or ovarian cancer (Singh J et al. Breast Cancer Res Treat, 2018 Jul;170:189-196). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.

This sequence change replaces aspartic acid, which is acidic and polar, with glycine, which is neutral and non-polar, at codon 750 of the CDH1 protein (p.Asp750Gly). This variant is present in population databases (rs759608783, gnomAD 0.03%). This missense change has been observed in individual(s) with colorectal, breast and/or ovarian cancer (PMID: 28135145, 29470806). ClinVar contains an entry for this variant (Variation ID: 231716). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

Familial cancer of breast Uncertain:1

Collection Method:clinical testing

The missense variant c.2249A>G(p.Asp750Gly) has been submitted to ClinVar as a Variant of Uncertain Significance, but no details are available for independent assessment. The p.Asp750Gly variant is novel (not in any individuals) in 1000 Genomes and has allele frequency of 0.004% in gnomAD database. The amino acid change p.Asp750Gly in CDH1 is predicted as conserved by GERP++ and PhyloP across 100 vertebrates. The amino acid Asp at position 750 is changed to a Gly changing protein sequence and it might alter its composition and physico-chemical properties. For these reasons, this variant has been classified as Uncertain Significance (VUS).
