Variant summary
Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PM1PM2PM5PP3_StrongPP5_Very_Strong
The NM_000512.5(GALNS):c.280C>G(p.Arg94Gly) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R94L) has been classified as Likely pathogenic.

Mucopolysaccharidosis, MPS-IV-A Pathogenic:2
In vitro functional studies supportive of a damaging effect on the gene product (low to null in vitro enzymatic activity; PS3_supporting); the prevalence of the variant in affected individuals is significantly increased compared with the prevalence in controls (PS4_moderate); absent from gnomAD v2.1.1 (PM2_moderate); multiple lines of computational evidence support a deleterious effect on the gene (PP3_supporting)
Morquio syndrome Pathogenic:1
Variant summary: GALNS c.280C>G (p.Arg94Gly) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant was absent in 247782 control chromosomes. c.280C>G has been observed in individual(s) affected with Mucopolysaccharidosis Type IVA (Morquio Syndrome A) (examples: Tylki-Szymanska_1998, Ogawa_1995). These data indicate that the variant may be associated with disease. A different variant affecting the same codon has been classified as pathogenic by our lab (c.281G>T, p.Arg94Leu), supporting the critical relevance of codon 94 to GALNS protein function. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect results in <10% of normal activity (Ogawa_1995). The following publications have been ascertained in the context of this evaluation (PMID: 7795586, 9660054). ClinVar contains an entry for this variant (Variation ID: 705). Based on the evidence outlined above, the variant was classified as pathogenic.
