Variant summary
Our verdict is Pathogenic. The variant received 24 ACMG points: 24P and 0B. PS1_Very_StrongPM1PM2PP3_StrongPP5_Very_Strong
The NM_001042492.3(NF1):c.667T>A(p.Trp223Arg) variant causes a missense change. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Another nucleotide change resulting in the same amino acid substitution has been previously reported as Likely pathogenic in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. W223C) has been classified as Uncertain significance.

Neurofibromatosis, type 1 Pathogenic:1
This sequence change replaces tryptophan, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 223 of the NF1 protein (p.Trp223Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with neurofibromatosis type 1 (PMID: 16944272, 29566708, 31370276). ClinVar contains an entry for this variant (Variation ID: 1754853). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt NF1 protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic. -
Hereditary cancer-predisposing syndrome;CN230736:Cardiovascular phenotype Pathogenic:1
The p.W223R variant (also known as c.667T>A), located in coding exon 7 of the NF1 gene, results from a T to A substitution at nucleotide position 667. The tryptophan at codon 223 is replaced by arginine, an amino acid with dissimilar properties. This alteration was identified in two individuals who met diagnostic criteria for neurofibromatosis type 1; and it was reported as de novo in one of these individuals (Griffiths S et al. Fam. Cancer, 2007;6:21-34 and Calì F et al. Eur J Med Genet, 2017 Feb;60:93-99). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
