Loss-of-function variants in NF1 are known to be pathogenic (PMID: 10712197, 23913538). This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with neurofibromatosis type 1 (PMID: 25074460). ClinVar contains an entry for this variant (Variation ID: 372756). For these reasons, this variant has been classified as Pathogenic. -

The W223X variant in the NF1 gene has been reported in at least one individual with clinical features of neurofibromatosis type 1 (Pasmant 2015). This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The W223X variant was not observed in approximately 6500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. Based on currently available evidence, we consider W223X to be a pathogenic variant. -

The p.W223* pathogenic mutation (also known as c.669G>A), located in coding exon 7 of the NF1 gene, results from a G to A substitution at nucleotide position 669. This changes the amino acid from a tryptophan to a stop codon within coding exon 7. This alteration was reported inan individual with neurofibromatosis (PasmantE et al.Eur. J. Hum. Genet. 2015 May;23(5):596-601). In addition to the information presented in the literature, since premature stop codons are typically deleterious in nature, this alteration is interpreted as a disease-causing mutation (ACMG Recommendations for Standards for Interpretation and Reporting of Sequence Variations.
