Variant summary
Our verdict is Pathogenic. Variant got 15 ACMG points: 15P and 0B. PM2PP2PP3_StrongPP5_Very_Strong
The NM_001042492.3(NF1):c.1062G>C(p.Lys354Asn) variant causes a missense, splice region change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. 3/3 splice prediction tools predicting alterations to normal splicing. Variant has been reported in ClinVar as Likely pathogenic (★★). Synonymous variant affecting the same amino acid position (i.e. K354K) has been classified as Pathogenic.

Alters the last nucleotide of the exon and is predicted to destroy the splice donor site but the effect on protein function is unclear; Not observed at significant frequency in large population cohorts (gnomAD); Reported in individuals with a clinical diagnosis or suspicion of neurofibromatosis type 1 referred for genetic testing at GeneDx and in published literature (PMID: 30290804); This variant is associated with the following publications: (PMID: 23656349, 18546366, 29673180, Douben2023[Functional study], 30290804) -
Not found in the total gnomAD dataset, and the data is high quality (0/276632 chr). Found in at least one symptomatic patient. Predicted to have a damaging effect on the protein. Predicted to negatively affect a known splice site. One de novo case with parental identity confirmed. -
Neurofibromatosis, type 1 Pathogenic:1
This sequence change replaces lysine, which is basic and polar, with asparagine, which is neutral and polar, at codon 354 of the NF1 protein (p.Lys354Asn). This variant also falls at the last nucleotide of exon 9, which is part of the consensus splice site for this exon. This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of neurofibromatosis type 1 (PMID: 30290804; internal data). ClinVar contains an entry for this variant (Variation ID: 586168). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. This variant disrupts the c.1062G nucleotide in the NF1 gene. Other variant(s) that disrupt this nucleotide have been determined to be pathogenic (PMID: 10712197, 18546366, 29673180, 31766501). This suggests that this nucleotide is clinically significant, and that variants that disrupt this position are likely to be disease-causing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -
