The NF1 c.2509T>C; p.Trp837Arg variant is reported in the literature in at least two individuals with a clinical diagnosis of NF1 (Alkindy 2012, van Minkelen 2014). This variant is absent from general population databases (1000 Genomes Project, Exome Variant Server, and Genome Aggregation Database), indicating it is not a common polymorphism. The tryptophan at this position is highly conserved across species and computational algorithms (PolyPhen2, SIFT) predict this variant is deleterious. Additionally, other variants at this codon (c.2509T>A, p.Trp837Arg; c.2509T>G, p.Trp837Gly) have been reported in individuals with NF1 (Alkindy 2012, Sabbagh 2013). This missense change has been observed in individual(s) with neurofibromatosis type 1 and breast cancer (PMID: 36612057; internal data). ClinVar contains an entry for this variant (Variation ID: 618753). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt NF1 protein function with a positive predictive value of 95%. This variant disrupts the p.Trp837 amino acid residue in NF1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 23913538, 31776437). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing.
