This sequence change replaces methionine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 1440 of the NF1 protein (p.Met1440Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with NF1-related conditions (PMID: 21520333). ClinVar contains an entry for this variant (Variation ID: 837385). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt NF1 protein function with a positive predictive value of 95%. This variant disrupts the p.Met1440 amino acid residue in NF1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 24789688, 25074460, 28961165; internal data). The NF1 c.4382T>G variant is predicted to result in the amino acid substitution p.Met1461Arg. This variant is also referred to as c.4319T>G (p.Met1440Arg) in an alternate transcript (NM_000267). To our knowledge, this variant has not been reported in the literature. However, different missense variants affecting this amino acid (referred to as p.Met1440Val, p.Met1440Thr, p.Met1440Lys) have been reported in multiple individuals affected with NF1-related disease (Xu et al. 2014. PubMed ID: 24789688; Bonatti et al. 2017. PubMed ID: 28961165; Pasmant et al. 2015. PubMed ID: 25074460). This variant has not been reported in a large population database (http://gnomad.broadinstitute.org), indicating this variant is rare. In ClinVar, this variant has been interpreted as likely pathogenic (https://www.ncbi.nlm.nih.gov/clinvar/variation/837385/). Although we suspect that this variant may be pathogenic, at this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -

The p.M1440R variant (also known as c.4319T>G), located in coding exon 32 of the NF1 gene, results from a T to G substitution at nucleotide position 4319. The methionine at codon 1440 is replaced by arginine, an amino acid with similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
