Genes affected
NF1 (HGNC:7765): (neurofibromin 1) This gene product appears to function as a negative regulator of the ras signal transduction pathway. Mutations in this gene have been linked to neurofibromatosis type 1, juvenile myelomonocytic leukemia and Watson syndrome. The mRNA for this gene is subject to RNA editing (CGA>UGA->Arg1306Term) resulting in premature translation termination. Alternatively spliced transcript variants encoding different isoforms have also been described for this gene. [provided by RefSeq, Jul 2008]

Uncertain significance, criteria provided, single submitterclinical testingInvitaeOct 08, 2023This sequence change replaces threonine, which is neutral and polar, with methionine, which is neutral and non-polar, at codon 1787 of the NF1 protein (p.Thr1787Met). This variant is present in population databases (rs760649828, gnomAD 0.003%). This missense change has been observed in individual(s) with neurofibromatosis type 1 and breast cancer (PMID: 16835897, 30287823). ClinVar contains an entry for this variant (Variation ID: 229848). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt NF1 protein function. Experimental studies have shown that this missense change does not substantially affect NF1 function (PMID: 22105171, 27791021). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Hereditary cancer-predisposing syndrome;CN230736:Cardiovascular phenotype Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsFeb 10, 2023The p.T1787M variant (also known as c.5360C>T), located in coding exon 37 of the NF1 gene, results from a C to T substitution at nucleotide position 5360. The threonine at codon 1787 is replaced by methionine, an amino acid with similar properties. This alteration has been reported in a 33 year old Chinese male with a clinical history suspicious for neurofibromatosis type 1 (Lee MJ et al. Hum. Mutat. 2006 Aug; 27(8):832). This alteration was also observed with an allele frequency of 0.00014 in 7,051 unselected female breast cancer patients and was observed with an allele frequency of 0.00009 in 11,241 female controls of Japanese ancestry. In addition, it was noy observed in 53 unselected male breast cancer patients and was observed with an allele frequency of 0.0001 in 12,490 male controls of Japanese ancestry (Momozawa Y et al. Nat Commun, 2018 10;9:4083). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
Hereditary cancer-predisposing syndrome Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsJan 02, 2015Thep.T1808Mvariant (also known as c.5423C>T) located in coding exon 38 of theNF1gene, results from a C to T substitution at nucleotide position 5423. The threonine at codon 1808 is replaced by methionine, an amino acid with similar properties. This alteration was first reported in a 33 year old Chinese male suspicious for neurofibromatosis type 1 (Lee MJ, Hum. Mutat. 2006 Aug; 27(8):832). This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.003% (greater than 30000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.T1808M remains unclear. This alteration has also been designated as p.T1787M(c.5360C>T) in published literature. -
